Finger printing Requirements |

You will be required to bring a photo ID and something that reflects your current address. If your current address is not listed on your photo ID you will be required to provide some proof of address by using some other form such as: electric bill, car insurance form, pay stub, checking account statement, student ID or any other type of form that will identify your address. We can not fingerprint any one without proper identification.
In the event you need to make alternate arrangements (click here) to view a list of community fingerprinting sites. There may be a fee for the actual taking of the fingerprints depending on where you have the fingerprints taken.
ADDITIONAL INFO YOU NEED TO KNOW:
You will need to complete a fingerprint card (these are available in our office) and return the completed fingerprint card to the provider who is responsible for submitting the fingerprint cards to OCFS.
The completed fingerprint cards will be sent by OCFS to the New York State Division of Criminal Justice Services (DCJS) and will be used to search their agency files for any criminal record of charges or convictions in New York State. There is no fee to you for the searches performed by DCJS.

If you have been convicted of a felony at any time for a sex offense, crime against a child, or a crime involving violence, or a felony conviction within the past five years for a drug-related offense, your application will be denied; or your license or registration denied, limited, suspended, revoked, rejected or terminated; or your employment terminated unless OCFS determines that approval of the application; continued operation of the center, program or home; or continued employment will not in any way jeopardize the health, safety or welfare of the children in the center, program or home.
If you have been convicted of any other crime, OCFS may deny the application if OCFS finds that the nature of the offense is related to the provision of child care. Prior to making a determination to deny an application OCFS will afford you an opportunity to explain, in writing, why the application should not be denied. That opportunity is provided as part of your application. You should be certain to include in the relevant place in the application a complete explanation of why you believe the conviction should not prohibit you from providing care for children.
Fingerprints are retained by DCJS until such time as you are no longer an applicant, operator, employee or volunteer of a child care program. All criminal history record information is confidential. It may not be made available for public inspection. However, nothing shall prevent a child day care provider or OCFS from disclosing criminal history information at any administrative or judicial proceeding relating to the denial or revocation of an application, employment, license or registration. |
